When should we bring in an external investigator?
When the incident is serious or high-potential, when internal independence would be questioned, when the people involved report to whoever would otherwise investigate, or when the same type of incident keeps recurring. External investigation also helps where the outcome may be reviewed by a client, insurer or regulator.
Do you deal with the regulator on our behalf?
We prepare clear, factual documentation and can support your team through the process. Notifying the regulator is your obligation and we will tell you plainly where that duty sits, but we do not provide legal advice or legal representation.
Is this a blame exercise?
No. Investigations that look for someone to blame stop finding causes. We look at the conditions, decisions and system factors that made the incident possible.
